Summary:
4.11, table 4.1: the definitions of the tags 'element' and 'attribute' do not state what happens if they are false. The fact that they both default to false implies that by default nothing gets serialized! In practice the normal behavior (documented elsewhere in the spec) is to create both elements and attributes where allowed - I think the explanations should state that 'attribute' means 'serialize only as an XML attribute (not as an element)'. This is borne out by 4.11.7 which states "The XMI 'element' tag can be used to signal that attributes can be serialized only as XML elements."
And similarly for 'element'. In which case there should be the additional constraint that attribute and element cannot both be true. -
