FIBOFND11 — Incorporate the description of the new table notation and revise table structure for those that have not been revised in other issue resolutions

Some of the revisions made to the 1.0 FND specification, including those made by FBC, have revised the table notation included in section 10 of this specification. The new notation should be described in section 6, and the various tables that have not been revised by other resolutions should be updated accordingly.

Addition of new Clause 6.3, changes in other Clause 6 mterial, plus new tables in Clause 10

This proposal covers all the works required for resolution to number 42 (originally in the accident-prone #52 which may now safely be ignored or closed))
