CORBA24 — POA exception minor codes

Summary: There are several cases in the POA specification where the POA is required to
raise system exceptions. We should assign OMG minor exception codes to
each of these cases so that applications can diagnose these conditions
better.

Examples:

1. POA has the USE_DEFAULT_SERVANT policy but no servant is assigned, the
POA raises OBJ_ADAPTER.

2. If no adapter activator is available (or the available one refuses to
create a POA), the OBJECT_NOT_EXIST exception is raised.
