CORBA23 — ORB::shutdown again

    Summary: I"m sorry to reopen this can of worms, but I believe that the shutdown()
    issue we voted on (1665) still only offers a partial solution. The problem
    appears to be that terminating the event loop and ORB destruction must be
    separate steps.

    Consider a very simple application that uses RETAIN, NO_IMPLICIT_ACTIVATION,
    and USER_ID. No servant manager is used.
