C2MS11 — Add DESTINATION-NODE and DESTINATION-FACILITY as fields

One goal for consistency (and ease of implementation and use of said implementation) is to have all subject elements also be fields in the message. Adding DESTINATION-COMPONENT late in the adoption process of C2MS was a step in this direction, but DESTINATION-NODE and DESTINATION-FACILITY are needed also. See page 82 for example for CFG messages.
