C2MS11 — Add field for USER to Message Header

In Log, Directive Request, and Simple Service Request, there is a USER field for use by communicating applications. This field should be moved to the Message Header, so that any message can utilize this field.

Note: in the Log Message this field is used in the subject, so is defined as a Header String. In the Directive and Simple Service messages, it is not used in the subject so is defined as a string.

Note 2: There is another field in the Message Header called USER-NAME; this is a tracking Field, so is reserved for use by the implementation and is the account/user name that started the application.

Move to C2MQ

This is really part of the C2MQ effort, so rather than add it to C2MS only to later move it to C2MQ is not preferred.
