C2MS11 — Clarify the ".... Message Header Notes" tables re: being included in each message

The ".... Message Header Notes" tables can be confusing. For example, table 8-8. It should be made clear that the fields listed in those tables are from the Message Header definition, but the values apply only for that specific message. A small wording change would go a long way to helping, especially for newer users.
